New Patients
Welcome to Animal Care Center Veterinary Clinic! If your pet has never been seen at our office, please take a few moments to provide us with a little information so that we may get your pet's file started. There are three options for doing this:
1. Fill out a form at our office when you come in for your appointment.
2. Follow this link to the New Patient Form, and fill it out and submit it online.
3. Follow the link and print the form out, fill it in, and either fax it to us or bring it in with you at your appointment. Our fax number is 503-371-3900.
When you're ready to set up an appointment, just call or e-mail us and we will be more than happy to schedule a visit for your pet!
